Autism Is More Physical Than We Think: What the Research Is Beginning to Reveal

When most people think about autism, they think about communication differences, sensory processing, and social interaction. But what if autism could also help explain chronic pain, persistent fatigue, dizziness, digestive issues, and joints that never seem to feel stable?


Over the past decade, research has been reshaping our understanding of autism. Rather than viewing it solely as a neurodevelopmental condition, scientists are increasingly recognising that, for many people, autism is associated with differences that extend well beyond the brain.

One of the strongest and most fascinating areas of research is the connection between autism and joint hypermobility, including Hypermobility Spectrum Disorder (HSD) and hypermobile Ehlers-Danlos syndrome (hEDS).


Understanding this relationship isn't simply about adding another diagnosis. It's about recognising that the nervous system, connective tissue, movement, and overall health are deeply interconnected.


The Evidence Is Growing


For years, clinicians noticed that many autistic people also presented with joint hypermobility and connective tissue disorders. What was once considered anecdotal is now being supported by stronger evidence.


In 2025, the first systematic review and meta-analysis investigating the relationship between autism and hypermobility analysed data from 20 studies involving thousands of participants.


The researchers found that:


  • 22% of autistic individuals had generalized joint hypermobility.

  • When assessed through a clinical examination rather than questionnaires, this increased to 31%.

  • Around 28% met the criteria for Hypermobility Spectrum Disorder (HSD) or an Ehlers-Danlos syndrome (EDS), rising to 39% in clinically assessed populations.

  • Most of the studies included reported a statistically significant association between autism and hypermobility.


These findings suggest that this overlap is far more common than previously recognised and deserves greater attention in both research and clinical practice.


Hypermobility May Be One Piece of a Much Bigger Puzzle


What makes this research so interesting is that many of the physical health conditions commonly reported in autism are also frequently seen alongside hypermobility. These include:


  • autonomic nervous system dysfunction, including Postural Orthostatic Tachycardia Syndrome (POTS)

  • chronic pain

  • persistent fatigue

  • gastrointestinal disorders such as IBS and reflux

  • migraine

  • pelvic floor dysfunction

  • differences in proprioception and motor coordination

  • sleep disturbances


Rather than seeing these as separate conditions that happen to occur together, researchers are beginning to explore whether they may reflect shared biological mechanisms involving connective tissue, the nervous system, and how the body regulates itself. This represents a significant shift in how we think about autism.


Why Might These Conditions Overlap?


Researchers are still working to answer this question, but several theories are emerging.


Connective Tissue Doesn't Just Support Joints


Connective tissue is found throughout the body. It provides structure for muscles, ligaments, blood vessels, the digestive system, skin, and many internal organs. Scientists are investigating whether differences affecting connective tissue development may also influence aspects of brain development and nervous system function. Although this research is still evolving, it offers one possible explanation for why autism and hypermobility appear together so often.


The Nervous System Is Central


The autonomic nervous system controls many of the functions we don't consciously think about, including heart rate, blood pressure, digestion, breathing, and temperature regulation. Differences in autonomic regulation are increasingly recognised in both autism and hypermobility. This may help explain why symptoms such as dizziness, digestive problems, fatigue, heat intolerance, and exercise intolerance frequently appear in both populations.


Movement Is More Than Muscle Strength


Many autistic people describe feeling disconnected from their bodies, struggling with coordination, or needing to think consciously about movements that seem automatic for others.

Hypermobility can make these challenges even more noticeable. When joints are less stable, muscles have to work harder to maintain control, and the brain receives less reliable information about where the body is in space. Over time, this can contribute to fatigue, muscle tension, reduced endurance, and a greater risk of injury.


Understanding these differences shifts the conversation away from being "clumsy" or "unfit" and towards recognising that movement may simply require different strategies.


Why This Changes the Conversation


For decades, autism has largely been discussed through the lens of behaviour, communication, and learning. While those areas remain important, they don't capture the full picture. Emerging evidence suggests that autism often involves multiple body systems, with hypermobility providing one possible link between many of the physical experiences autistic people describe. This perspective encourages clinicians, therapists, and researchers to ask different questions.


Instead of treating chronic pain, fatigue, digestive issues, or dizziness as isolated problems, we can begin to ask whether they are connected through shared underlying mechanisms. That shift has the potential to improve assessment, support, and quality of life for many autistic individuals.


Looking at the Whole Person
